Excusing Absences

For some important circumstances, the students can ask permission from the teachers to be excused from the class by filling this student leave form with the condition that the students will still fulfill the course requirements posted for the days that the students were absent.

The teacher can evaluate if the reason for the excuse is reasonable or not. (Acceptable reasons could be: family emergency, change of work schedule, being sick, death of a family member, and others).
